• Reorder fields

1. Rename "Submitted by" to "Survey name" and bring to the top
2. Group name as second option. Move the help text as well.
3. Make "Participating as" as the third option.

• Enable bulk upload of data to table.
• Test bulk upload is working.
• Add new bleaching category to table. The categories are

1. NB - No Bleaching
2. PBMI- Partial bleaching (minor)
3. PBMA - Partial bleaching (major)
4. FB - Fully bleached
