Qualys VM connector using unsupported Function Extension bundle version

The Qualys VM connector references a no longer supported function extension bundle in host.json. The function app should be updated to support v4.

@PCNZ, PR has been raised with the fix, once's it gets merged changes will reflect in package file. Just restart your function app after PR merge.
