FastAPI can fail to record App Service URL

FastAPI reports IP instead of URL in Application Insights table.

The issue is in the way the URL is being constructed in the asgi instrumentation in the opentelemetry-contrib repo. Raised a corresponding issue in that repo and will fix it as well.
