Wholesale cleanup of all history plugins

- remove the use of redundant cite
- minor cleanups to history.plugin.bash
- set explicit load orders for history-search and history-substring-search
- add new plugin history-eternal
- replace superfluous trap for managing HISTTIMEFORMAT changes
