Create a new feedback form #561
Description
As the SAMHSA team won't have access to Google suite after the 18F engagement ends, it's important to find another way of soliciting feedback from the public as they use the site. Collecting feedback has been a critical tool for us to recruit research participants and to see patterns in the feedback from the visitors in order to prioritize features for MVP.
This could be a form that is kept up all the time, or one that you use when new changes have been made to collect feedback and measure the success of that one new feature and how it affects the user experience.
For the near term, we recommend at minimum creating a form that's linked to in the footer. @BrooklynHopkins - I believe you were looking into options but any basic form (survey monkey, etc) that's approved by SAMHSA IT could work.
