Consider versioning Outcomes like Decision Points

It seems like we should consider versioning Outcome sets as well. This seems like a relatively easy thing to clone from what we did in #350 and #368 for decision points.

At least partially addressed by #672. Need to review to see if it's fully resolved or not.

Do outcome sets follow the same versioning rules as decision points, or as decision point groups? I guess outcomes are analogous to decision points and outcome sets are analogous to decision point groups?

The idea is that an OutcomeGroup is equivalent to a DecisionPoint, and Outcome values are the same as Decision point values.

There is no parallel for DecisionPointGroup on the Outcome side.
