Event Attendance Reports

Tested on Demo 4.1.2 EventAttendance.php

The initial event summary list shows correctly all available event types.
Links from that list appear to be incorrect:

http://demo.churchcrm.io/master/EventAttendance.php?Action=List&Event=1&Type=Church%20Service

same outcome is achivable with:
http://demo.churchcrm.io/master/EventAttendance.php?Action=List&Event=1

Clicking any of the above returns list of events in that category but with incorrect numbers of attendees, breaks left-hand menu functionality and wipes page footer.

The 'Generate Report' buttons show numbers, but they look random and are not linked to the numbers of attendees.
