update readme (change file-name convention)

The answer file which is to be created in 'javascript/answers/' should be named as 'github_username-q-question_number'.

For eg: If I am adding question 5 JS answer, then the html file which is to be created inside 'javascript/answers/' should be named as 'dakshkhetan-q5.html'
