DARIA super safety mode?

Jotting down an idea I had a weekend or two -- to send a pledge you need basically five fields (appointment date, clinic, name, phone, amount of money). Would there be any benefit to slimming down the UX to JUST those critical fields (modulo an extra few) and hiding everything else?

I think its a good idea. Are you thinking we keep or lose the note taking?
