ORCID Settings page layout is messy #4302

Describe the bug

After logging in via ORCID, and creating a "Researcher Profile", the ORCID Settings page has a messy layout. It's possible this is because the page wasn't fully updated when we upgraded to Bootstrap 5 in #3506

Here's what it looks like

To Reproduce

Steps to reproduce the behavior:

  1. This can be reproduced on https://sandbox.dspace.org
  2. Login via ORCID using the ORCID Sandbox (you must already create an account)
  3. After login to DSpace, go to the "Profile" page under your account
  4. Create a "Researcher Profile". Then click "View" to go to that Researcher Profile
  5. On the Researcher Profile page, select the "Open ORCID setting page" from the action menu.
  6. You'll see a screen similar to the screenshot above.

This can not be reproduced on demo.dspace.org

Expected behavior

The radio buttons & checkboxes should align with the options, as they did in DSpace 8.x. Look at the demo.dspace.org site for reference (perform the same steps above on the Demo site and the page looks fine).

Related work

I suspect it may be related to styling changes from Bootstrap 5 in #3506. It's possible this page is using older Bootstrap 4 styles.
