Created /tmp/plan_complete too early

Claude is just really good at being lazy.
Even though there were 12 phases, and he only completed 10 tasks in 2 of the first phases, he did this:

Another check should be done instead.

in ## Important add something like "- Do not output /tmp/plan_complete until ALL phases are complete"

also if the context gets too full it tends to delete the information so adding something like "- Exit out without emitting /tmp/plan_complete once you've done with a phase, unless you were working on the final phase."
