Add extension for session form for species entry

Current Implementation and Limitations

For the case of preclinical subject data (i.e. animals) It would be good to have the option of adding species to it. It would be best to add a mandatory form which precedes the session, which the group manager can select to reveal.

Add an option in group manager for to display a mandatory form to fill in that precedes the session form
If this option is selected add the form to the session form page to be filled out
On datahub either have this form info displayed in the table by extracting it from the record or have it in the usual subject table.
