Add action:death in lookups

Currently, the only way to query death entries is through `a:kill`
When a `user:` field is specified, the entries shown are where the user has killed a player/entity

This makes it impossible to find death entries if we only have the victim's username, or in rare cases where the player was killed by `#falling_block` or something
