MAPL_CFIORead is being depreciated

We are depreciating MAPL_CFIORead and replacing with a better routine MAPL_read_bundle that is based on newer software and consistent with the IO layer used by History and ExtData as well as fixing all the problems and inconsistencies in MAPL_CFIORead.
Also noticed that cl2_offline (which uses MAPL_CFIORead) is not being built so should be removed as it is almost certainly broken.
