Insurgency Server not starting

Not sure why but in the last week my Insurgency servers (not Sandstorm) refuses to start and I can not see any errors. I have made no changes to the server recently so not sure what has gone wrong.

I have updated lgsm, updated the game, ran validate but still not able to start my server.

Any help will be greatly appreciated.
