Create integration test table before running the test

DataBoostIntegrationTest currently relies on a table called DO-NOT-DELETE-cbt-data-boost to exist when running the test. We should try to avoid having pre-existing resources for the test if possible or, failing that, document what are the requirements for running the integration test
