Table of Contents - cannot be deleted, does not update automatically, does not have update button

When inserting a Table of Contents at the top of the document, it does not behave as expected:

1. it does not auto-update as new headings and sections are added
2. it does not have an "Update" button revealed on hover or right-click to update table as new headings/sections updated or added
3. it cannot be deleted - for some reason i can add another table of contents but cannot delete the old one

The above issues, the last of which being the most annoying, make the feature and document in its entirety unusable. I am going to have to copy > paste into a brand new document and delete this one.
