Refactor script parameterization and add output options

Expose script parameters as optional input on commandline or a
configuration file. Enable the user to control the number of processes
and outputs.

Add option for saving a summary image composed of thumbnail versions
of all images (3D images represented by maximum intensity projection
along user specified axis).

Improve identification of DICOM series beyond the use of seriesUID and
studyUID, including user specified tags for associating a file with a
specific series.
