Support for Custom base URL for OpenAI and Anthropic Providers

When using BYOK allow configuring JUNIE_OPENAI_BASE_URL and JUNIE_ANTHROPIC_BASE_URL.

This allows Junie to be used with internal LLM gateways such as LiteLLM.
