fixes #195 This really needs some tests as the non-fast_scalar_indexing path was just completely broken.

Can you add a test? It should be able to run with the Buildkite systems now. |

Do we have a good Cuda surrogate array package? Using a GPU for testing this seems sub-optimal. |
