MathQuill has removed support for indefinite integrals

MathQuill has removed support for indefinite integrals, making integration limits mandatory as of pull request mathquill/mathquill#627, commit mathquill/mathquill@7932a34.

This is not a concern at the moment. There is no evidence that we need indefinite integrals.
