Description
The order of the last two variables should be reversed. That is, the following header:
fips,county,state,npi,start_date,end_date,citation,note,re-opening_note,re-opening_citation
should be changed to:
fips,county,state,npi,start_date,end_date,citation,note,re-opening_citation,re-opening_note
