Things to include:

1. Module Name as title
2. Module Case Type as subtitle, small
3. Module Status(on if any of the forms is on)
   3.1 If module on, all forms are set to on
   3.2 Defaults for forms? Should each form have its own default or per module?
4. Number of forms
5. Number of forms on
6. Number of forms off
7. Data collection window(run until window expires, then turn-off. Ability to change window)
8. Data collection frequency(monthly,quarterly,bi-annual.. Should we allow for cron expressions, maybe via OpenFn? )
9. Date turned-on
10. Date turned-off
11. Turned-on/off by
12. Switch to turn-on/off module/form. Should be inline with module title or form title. Show anim while turning-off/on
