Overide translation extension

I'm attempting to follow the instructions here: https://doc.livehelperchat.com/docs/language#how-to-override-default-translations

The first thing it states is to "Create an extension with the following structure" and then gives some text, which I'm assuming needs to be inserted into file somewhere. Please could someone assist me in creating this extension and/or where to add the text?

Just in case anyone else is as silly as me - I've worked out that the text isn't to go in a file, it's a explanation of the required folder structure.
