Please update use changes from forks

I hove you will update changes from forks some time, like using oracle and other changes. I hope Oracle with live in separate dll MiniProfiler.NHibernate.Oracle because it needs dependency on proprietary dll. Update infrastructure to be more generic as forks suggest.

Also create 2 version of output one is for 4.0 other for 4.5. I see that 4.5 fork adds Azure support. Please update this project to make it usable.

The Oracle fork wasn't in a state I could merge last time I checked.

Definitely happy to bring in changes from forks, but ideally the owners would raise those as pull requests so that we can discuss / merge them in.
