fix: remove dead code and add missing reflection continuation logic

- Remove unreachable code after while True loop (lines 1414-1429)
- Add missing reflection continuation code for non-satisfactory reflections
- Fix unnecessary f-string prefix in error message
- Fixes syntax error preventing test imports
