Remove j loop ex_coef #216

Brief description
The boundary layer is forked in LFRic, and due to data structure changes in LFRic, is not performent with threading.
Remove the j loop and improve blocking behavior in ex_coef by hand.
PSyclone was considered to be used, but required fixes are not due until April (at the earliest), and as I've done the work for performance and KGO testing, we should use it.
Full removal of j index to follow in another issue upon request from Chris.
