Bug: `edit_file` tool using `delete`, `find_replace` modes formatter has a confusing output after initial tool acceptance

Currently, delete and find_replace modes output a + where the diff took place, this is confusing as it looks like an addition and doesn't look like the initial diff that Nanocoder asks for permission on.
