Create a facility view on the dashboard

The facility view should display the state name, district, facility type, facility name, nin number as a header.

Below this display, the list of assessments done for the facility. In this display assessment start date, end date, assessment type, assessor's name, and other metadata stored during submission.

Each assessment should have a link that takes one to the embedded Metabase dashboard from where the user can download the assessment as CSV.
