Remove Swizzle2D

IIUC, we are not using Swizzle2D anymore. This is one of the last few cases where indexing needs to fall back to the legacy code as it isn't supported with TensorIndexer. Any objection to remove it?

The latest TC instruction are only supported on B300, B200, and H100. All other blackwell and hopper generation cards only support SuperMMA like Ampere.
