NethVoice CTI: Missing some fields adding a new contact in NethVoice CTI

Create a mockup for the parent issue: #7124

When I add a new contact to the address book in NethVoice CTI I find this insertion modal

I cannot add different fields provided in the address book (for example address, site, etc.)

The proposal is to maintain this structure but to give the possibility to select one or more fields to be still able to insert the data.
