There seems to be a problem with Optifine Custom Items textures when the models use textures that are not in the basic subfolders of the "textures" folder, such as "blocks", "items", etc. If one model used as CIT uses a texture that is in a different folder, even if the folder in question is inside one of the basic subfolders, such as "items<new_items", for example, such textures won't load, loading the typical magenta and black squares texture instead.
It doesn't matter if the item is in an item frame, an armor stand is wearing it or if you are holding it, just have it in your inventory or if it is lying on the ground.

PD: I've also noticed that if any of the item's texture is in the folder where the item model is (which is a common practice with Optifine CIT items), it won't load either.
