Suggest a Couple Additional Valuation Fields

Reviewing the information being collected currently, we might want to add:

1. Expiration Date (optional)
2. Report Date (optional) - sometimes different from the actual effective date

Comments from TWG Meeting 2/15/22:

1. Additional fields don't necessarily clarify things as they are subjective as well.
2. However, we want to make sure that it's extremely clear what "valuation_date" should signify (the effective date of the valuation). Suggest renaming to "effective_date"
