rx: refactor rx pipeline and transfer reassembly state machine

* bug fix: state machine would reject many single delimiter sequences
* inline state machine encoding into reassembler counter
* improve error handling
* improve MISRA compliance
* minor API improvements
* remove redundant InternalRxSession struct
