Revise to use Blacklist hook properly

Version 2.11.0.1 of the blacklist now allows html hooks. Lenny needs to be revised to properly use it, echo is not inside a table as expected.

html hook to Blacklist appears to work fine for FreePBX 2.11, but a report from a 2.10 user indicates issues with Lenny setttings getting removed when adding a number to the Blacklist. Ver. 0.0.6 temporarily removes the hook until I can figure out what is going on.
