[Observation Log] Manual log to capture failed RavenDB setup actions

Outcome of #25.

This issue exists to capture cases when RavenDB fails to complete setup, but does not throw an error, causing errors downstream. Please create a new comment for each instance and add relevant logs downloaded from the actions page (do not just paste a URL link because those expire in 90 days and may become inaccessible before review).
