Add g-stream monitoring of cache activity

This pushes out aggregate g-stream information for the S3Cache object;
ideally, this will allow more careful understanding of the performance
of the cache itself.

Additionally, for testing sake, makes the cache entry size configurable;
if the cache entry size is set to 0, all the reads bypass the cache.
