Issue #75 follow-up

Okay, it transpires that DPAE::Provider::Database depends on two extra columns in its user table, ```pw_reset_code varchar(32)``` and ```email``` (the address to send the email to). I suspect this is more of a documentation bug for DPAE::Provider::Database.
