Moodle questionnaire plugin - accessibility issues

1. Inconsistent Link/Button Styling in the questionnaire activity. Within the questionnaire, links are styled differently (some look like buttons, others like plain links). This can confuse users, especially those using voice input.

2. Ambiguous Label. “Print Blank” label unclear and not descriptive and tooltips are not accessible via keyboard.

3. Questionnaire error messages appear in multiple places on the same page (top and bottom), the bottom error message references question numbers that are not visible due to branching logic.
