Bug: Workflow Actions Bug

Problem

When you create a workflow, and start creating actions for that workflows, the actions after atleast 2 don't properly show all of the actions for that workflow, either one of the previous ones get deleted in the UI.

Steps to Reproduce

1. login as scoopdinator, and go to workflows
2. add workflow
3. add actions to this workflow maybe 3-4
4. now after createing 3-4 actions you will only see 2 workflow actions there.
