Bug: Root Action issue

Problem

When you create a workflow, it automatically creates a root action for that workflow given its name, and also an actions with the same name. and this also shows in the bubbles, where if you assign this workflow to other users, and the users go to bubbles they will see root action and also the action, even though they are the same thing.

Steps to Reproduce

1. login as scoopdinator
2. create a workflow
3. assign a user to that workflow
4. login as the assigned user
5. go to bubbles
6. you will see 2 actions even though there should be 1 (or none, because technically the scoopdinator hasn't created a action yet.)
