Variables with both a quantitative and qualitative result

How should we handle variables with both a qualitative and quantitative result? For example, AHI can be given in events/time or as a category. Should we allow for both to be reported? Would a single MeasurementObservation have both a value_decimal and value_enum?

Qualitative results I would place in either MeasurementObservation.value_string or MeasurementObservation.value_enum depending on whether or not the value is constrained. Quantitative results I would place in MeasurementObservation.value_quantity.

If a participant has both qualitative and quantitative results, I would create separate MeasurementObservation records for each result.

Even in the following example?
I have results of a blood test that are 5.7 mg/dL - that translates to a value of "high". It's the same lab with two ways of presenting the same results. How would we distinguish between multiple labs and one lab with results presented two different ways?

AHI, CESD score, and BMI
There's a number either integer or decimal and that number is assigned a category. We could receive both or only one in the incoming data set. I also worry that some of these numerical values might be assigned a category (like normal or abnormal) based on other metadata that we may or may not receive, such as age, sex, race, whatever.
