UI issue on mac browser screens for extension request modal

Issue Description

On smaller screens, the extension request modal breaks, with various elements overlapping.
The issue seems to be present only on safari browser/iPhones.

Expected Behavior

The modal should be responsive for smaller devices as well, and the prevent overlapping of elements.

Current Behavior

When a new extension request is raised, the modal that shows up is not responsive on smaller screens, with texts and elements overlapping over each other

Steps to Reproduce

1. Open the my site on a smaller screen device like (iPhone 8)
2. Open the list of extension requests(create one if you don't already have), and click on extension status.
3. The modal that opens up, is not responsive (transparent background and overlapping elements)
