"API Request Cancelled" occurs on any and all requests

🔁 Steps to Reproduce

I am running Windows 11.
I have uninstalled and reinstalled VS Code, uninstalled and reinstalled Roo Code, and disabled all other plugins.

I can make no API requests at all. Every attempt to submit a request starts with Roo displaying the usual "API Request" with a spinning indicator, but that is only visible very briefly as it is almost immediately replaced with the message with "API Request Cancelled". It seems that nothing is even attempting to send.

I have multiple API Providers configured (Google Vertex, AWS Bedrock, OpenRouter) and they were all working previously until this error started occurring. I have also tried different modes.

💥 Outcome Summary

Expected API Request to be sent to API Provider, but got back error stating "API Request Cancelled".

PR #5428 has been created to address this issue: #5428

This fix resolves the root cause where API failures were being misclassified as user cancellations, causing the misleading 'API Request Cancelled' message to appear for all API/network issues.

I am having this as well and the weirdest thing, it seems only happen to VSCode, I also install Cursor and Void, both of them are fine.
