🔁 Steps to Reproduce

Start with Orchestrator, have it perform sub-tasks. Sometimes those subtasks get interrupted (ie Claude overloaded error), try to exit subtask and go back to main task, main task not using orchestrator anymore. Instead the main orchestrator task is using the sub-task mode like code or architect

Also, I think I see issues where modes except code have a lower chance of returning to the parent task after they are done. I've seen many times where the architect mode or ask mode do not return to the parent task after completion.

I have successfully implemented a comprehensive fix for the orchestrator mode restoration issue and created pull request #5748.

🔧 Solution Summary

The fix addresses the core problem where sub-tasks launched by the Orchestrator mode were not properly returning to the parent Orchestrator task after completion or interruption. I implemented a comprehensive solution with four major components:

1. Enhanced mode restoration in resumePausedTask method

• Added comprehensive mode restoration logic in Task.ts lines 766-793
• Checks if current mode differs from paused mode and restores it with proper error handling and logging

2. Mode restoration safety net in finishSubTask method

• Enhanced ClineProvider.ts lines 229-264 to include mode restoration as a safety net
• Ensures parent task's mode is properly restored regardless of how the sub-task ends

3. Mode validation and restoration in task resumption from history

• Added mode restoration logic in Task.ts lines 814-838
• Ensures correct mode is restored when tasks are resumed from history

4. Error handling for interrupted sub-tasks

• Enhanced error handling in Task.ts lines 1462-1520 to include mode restoration
• Ensures parent task mode is restored even during error scenarios

Hey, i am facing this same issue. Also it feels like this got consistently worse. A few weeks ago this wasnt a huge issue (had like 1 or 2 subtasks for multiple orchestrator sessions that failed) but now for a single sessions multiple sub-tasks may fail to return properly.

I also noticed this happens with the code mode (in contrary to the author who didnt notice this yet in code mode). What i also noticed is that modes claim they used the "attempt_completion" tool correctly when asked to use it again and retry but fail again. So i am not sure if they are using the tool wrong, if they are just forgetting to use a tool at all and just using chat completion or what they are doing, but its something i noticed.

This is also an issue in normal tasks without interruption (using claude code, might be an issue here) or anything interferring.
