Model is hallucinating and finding files which it isn't there even when it has done code index also allow it to understand internal tool calling protocol, also allow it is condense content within its limit

Hey @M-505, I understand that it is frustrating having issues with the models, since Roo Code uses a pretty complex system, prompt some models might perform worse than others, especially smaller models like GLM 4.5.
