[BUG] auto-approved for running MCP tool is not working for v3.31.1

Problem (one or two sentences)

I used code mode to run an mcp tool with auto-approved config having running MCP enabled and MCP tools are marked as "always allow", however, Roo is still asking for permission

Context (who is affected and when)

Seem to be affecting all users using MCP tool with auto-approval on

Reproduction steps

1. Make sure you are in Roo v3.31.1
2. Switch to code mode
3. Enable auto approval for MCP
4. Mark your mcp tool "always allow"
5. Prompt Roo to use the mcp tool

Expected result

UI does not ask for permission

Actual result

The UI pauses Roo and is asking for permission even with auto-approval and always-allow enabled

Already fixed and will be released in 3.31.2

I noticed this issue was marked as fixed but the fix hadn't been merged yet. I've opened PR #9215 with the existing fix from the fix/mcp-tool-auto-approve-regression branch that addresses this issue.

The fix restores the expected MCP auto-approval behavior where enabling alwaysAllowMcp will auto-approve all MCP tools by default (unless a tool explicitly opts out with alwaysAllow=false).

All tests are passing and the implementation is ready for review. Feedback welcome!
