scsi: fnic: Add and integrate support for FDMI

Add support for Fabric-Device Management Interface (FDMI) by introducing
PCI device IDs for Cisco Hardware.

Introduce a module parameter to enable/disable FDMI support.

Integrate support for FDMI.
