Some information is not currently well handled in doctor declaration processing - DoctorDeclaration

Bug Description

Among other details that are not currently supported by the external message processing, some needed information is currently not stored when processing doctor declaration :

• Person: occupation not reported
• Person: Tutor is not reported, but "Person has a legal guardian" checkbox seems always checked (all diseases)
• Case Notification: date of diagnosis not reported in the "notification" box
• Case Notification: date of diagnosis not reported in the "notification" box
• Symptoms: date of onset not reported

Reproduction Steps

1. Process a DD message with specific details
2. Check details accordinlgy

Expected Behavior

The information that is received from the doctor declaration should be reflected in external message and in turn be recorded when the message is processed.
