Bug Description

Treatment started field is currently stored in Therapy entity which may be subject to specific user rights.
Additionally treatment started may be derived from other entries from the therapy.
Doctor declaration/phone notification currently make use of this field.

Reproduction Steps

Expected Behavior

Treatment started should be either stored on Case entity or derived based on Case Reports or aggregated from Case Reports/Therapy entries
